Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family. The home health agency staff will also talk to your doctor about your care and keep your doctor updated about your progress.
RN PCU
We are seeking experienced clinicians who are eager to progress their career within the home health industry. Participate in PHPC's orientation and in-service training programs for professional staff. Share in providing 24-hour, seven-day-a-week coverage to patients/families when necessary.
These are available for a maximum of 10 hours per month and are covered by a grant through the Office for People With Developmental Disabilities. If you have a Medicare Supplement Insurance policy or other health insurance coverage, tell your doctor or other health care provider so your bills get paid correctly. Their quality of life is negatively impacted when they are unable to get the correct treatment for their mental and emotional diseases. It can be challenging for people with I/DD who are struggling with mental illness to ask for support. Comprehension issues could make it difficult for them to find the right programs and practitioners to help them.
